ABOUT YOUR SAFETY AND PRIVACY

IS IT SAFE TO PARTICIPATE IN THIS STUDY? 

Some of the participants in this study will participate in an exercise program. We will assess your heart function and aerobic fitness using a walking treadmill test and electrocardiogram (EKG). This test will help determine whether you are safe to become more physically active as part of your participation in the MEDEX study. The walking treadmill test and EKG will be conducted in a laboratory setting under the supervision of a medical resident physician as well as an experienced technician. With your permission, we may also talk to your doctor before you do this test.  The study team includes doctors, nurses, psychologists, social workers, physical and occupational therapists, and exercise scientists. We are committed to helping older people stay healthy, both mentally and physically. We will also take every care to ensure the safety of participants.


HOW DO YOU PROTECT THE INFORMATION I WILL BE SHARING?
Research records will be kept confidential to the extent allowed by law. To help protect your confidentiality, we will store all paper documents in locked file cabinets within a locked office. Electronic records will be password-protected. No information that could identify you will be used in papers or presentations about this study without your written consent.


WHO HAS ACCESS TO MY HEALTH INFORMATION ASIDE FROM THE MEDEX TEAM?
With your permission, we may contact your doctor about your eligibility to participate as well as any concerns that arise throughout the course of the study. Your health insurance will not be affected by your participation in this study.


CAN I WITHDRAW FROM THE STUDY WITHOUT PENALTY OR LOSS OF BENEFITS?
Participation in this study is entirely voluntary. You may refuse to participate or withdraw at any time without penalty or loss of benefits to which you are entitled. If you decide that you no longer wish to continue in this study, you will be requested to come in for a final visit and return study materials. You will be told if any important new information is found during the course of this study that may affect your wanting to continue.
